End of Life Planning empowers Sangha to plan for their preferred personal and spiritual care during their last stage of life. Social workers will facilitate the Sangha to discuss and document what they need and their last rites. We will also assist the Sangha to complete their Will, Lasting Power of Attorney, Advance Care Planning and Sangha Living Will if they wish to do so.
